I saw a small piece of duct tape cost the Air Force a bundle of cash years ago at Tachikawa AB, Japan. A vent in a C-130 wing tank was covered with tape for a procedure and was inadvertently left on afterwards. When the plane was fueled the piece of tape held well enough that the wing ruptured and fell off the plane from internal pressure.
