My usual response, because I don’t like the automatic “fine”, or something enthusiastic like “GREAT!!!!”, is “could be worse”. For some reason, people seem to think that I mean this to be funny, and for some even more baffling reason, many people seem to think it actually is funny. Oh well. I’ll never understand people.

My response to “how are you doing?” is frequently, “I’m doing,” or maybe “I’m here.” People may interpret those answers however they wish.
