My dad (circa 1970s) had some surgery and the meds made him unruly. I got a call to come and get him because they didn’t want him in the hospital. Luckily Mom and I could take care of him for a day or so.
As a child I had a tonsillectomy. Ether was the anesthetic, I can still “memory-smell” it decades and decades later. The O.R. staff thought I was under, but they were wrong. As a result, I fetched one of the nurses a rattling kick in the breadbasket. Then they “poured it on” until I was well and truly ethered. Boy, did I have a bodacious hangover when I woke up after surgery!
Here’s a description of a 17th century surgery to remove a large bladder stone without anesthesia. Although it doesn’t say so in the linked article, other descriptions I’ve read of the same surgery said that the doctor had enough helpers to hold the patient down as he struggled.
