I’m not sure why this is supposed to be funny. Skipping pills can have serious consequences.
I think “P.M.’s” refers to a sleeping pill.

Since the clock seems to say 11 p.m., I guess this might mean Burl takes P.M’s ‘cause he wakes up at 2 a.m. and can’t get back to sleep, annoying the heck out of Joy (hmm, Burl more than ever resembles a nice soft-boiled egg without his glasses)
Perhaps Tylenol P.M. or Advil P.M. which are combo pain relief and sleep aids.
