Example 1: A precision coating process for military optics specified using non-expired chemicals. Reality: three years past expiration date. Example 2: Integrated circuit lithographic imaging process required positive room air pressure for dust free environment. Reality: “The blowers have been broken for a while.” I could go on and on.
