Man: Look, doctor. Let's make a deal. If you call me buddy, or chief, one more time, I will cease the professional deference of calling you "doctor," and start calling you #**%#!! ok?
A friend of mine called this the “witch doctor syndrome” it has noting to do with medicine, but everything to do with enhancing the doctor’s status. It is also why I missed the birth of my children – back then they wanted the bonding with the doctor, not the family. My friend also would point out that he was on a first-name basis with several high-reputation PhDs in theology, history, and science, but no MDs.
And as the housewife told the doctor, her Mrs. took even more time to earn than his M.D. I see no reason for a doctor to fake familiarity with a patient. He needs to earn that, too.
