The flags are very common around here (NE Ohio), on both the hearse and the other cars (one flag) in the procession. Hearses or “lead cars” tend to have a blinking light of some kind (usually amber), too, At one time the local PD would provide the lead car.
You’re not supposed to pass vehicles in a procession, nor weave in and out of one.

It’s a US custom. Probably derived from the black plumes, but modified for automobiles.

Never seen one in Canada or US, all the ones I’ve seen use a purple flashing light.
