I was working at NASA in the late 60’s when we changed to Success Oriented Management where the motto was “Failure is not an option.” With this in mind, the Space Shuttle was the most fantastic spaceship ever designed but had no escape mechanism. They have gone back to the escape capsule which helps a lot but failure is definitely an option.
