Once, to try and keep the elevator doors open for someone, I hit the alarm button, which was close to the “doors open” button. Not clumsiness, bad design! :-)
The local mall’s escalators have emergency off buttons on a pedestal placed EXACTLY where a child would press them (and adults could do so accidentally). Duh! I’ve seen the repairmen and security permanently stationed there on busy days.

I’m thinking it’s more likely the self-destruct mechanism won’t work due to shoddy parts from the lowest bidder.

Reminds me of the “Wings Stay On” and “Wings Fall Off” buttons next to the volume knob on the airline jet passenger seat (Far Side).
