This is usually when a patient is taking so many drugs, there are interactions between certain meds..The purpose is to reintroduce some drugs to the patient and observe the changes. In most cases, the patient can be released taking fewer drugs that before, and actually feeling better.

My medications are easy to list: Aspirin for headaches, low dose aspirin for blood pressure.

Being a nurse for centuries, the longest list we see is the past/current conditions. Meds are easy compared to that.
