If you have time six months or so start by sailing to Naples/Pompeii go to Rome, Florence, Genoa, Turin, Milan, Verona, Padua, Venice, Parma, Bologna, San Marino, Pescara, Bari, Leuca, Gallipoli, Catania, Palermo, Syracuse then back to Naples and catch the boat home. If you have more tine go back to Rome.
From my own experience I’d say start in Rome, give it three days, then move on to Tuscany and these places. You’d have to be from a really big city in order to be able to stand Rome for much longer than a couple of days. Less in the summer.
