I wouldn’t worry. If it’s like the real paper towels the perforations aren’t deep and don’t go all the way across.
It may be only a slight perforation, but it recurs at fixed intervals. It’s a chronic condition. Probably infectious. Notice how the doc’s gloved up before touching it.
