OK, thanks to the links posted by mabrndt it is possible to call up and enlarge this work. It then becomes apparent that she is smoking – a long stemmed, small bowl, white pipe – probably white clay (I have seen other pipes of the sort). A wisp of smoke appears just over her lips on her left side. The two men behind her are also smoking, and the one directly behind appears to be loading a very similar long stemmed white pipe – which explains why it looks like he’s feeding a piece of string over her shoulder, the same ‘string’ which appears to lead from her mouth to hand is actually the pipe stem.

Gladys’ doctor told her to stay away from tobacco…so she got an extra long pipe.
