A long time ago I worked at a hospital that had their ICU in the basement. Lighting the same night and day, no wonder the patients got confused. They finally had a mural of the sun coming up (or setting) painted on a wall with lights that slowly got brighter and dimmed at the proper times. Not sure if most of the patients were able to appreciate it, but the staff definitely did.
