When I was “losing” teeth, we would tie a string around the tooth and a door knob. Then my older brother would quickly slam the door shut. The tooth always came out and then I could relax. Living in Germany at the time and there was no such thing as a Tooth Fairy. Getting the tooth out of my mouth was reward enough.
