In Wales (UK) people in small villages, years ago, often differentiated between the many people with the same surname by referring to them as, for instance, ‘Jones the milk’ ‘Jones the farm’ ‘Jones the taxi’, and so on. None of them objected, as far as I know.
I wonder how Ruthie would take to being called… Sanitarium Sister. (I was about to say ‘twisted’ but then remembered that I needed to use a place name.)

It’s not a bad idea to call people such names if you don’t know their name. At least everyone knows who you’re talking about.
