Well, I’m sure part of it was navigational problems of olden times, causing some sailors to loose track of their actual location and sometimes sighting what they think is new land but is in actuality already found land, the sailor is just off-course and not quite where he thinks he is.
Obviously, that changed with the eventual coming of modern navigational practices.
