I find that with a little nibbling away with a wire cutter is worth the trouble to remove the outer part of the cap and then just use the inner part with no bother for the rest of the life of the bottle. (And if you get another bottle as a refill, just reuse the modified cap from the previous bottle.)

You bang it against the table and rip it off with great force. Then you get your four year old to remove it when that doesn’t work either…

vise grips the wonder tool (and after they are done with it usually easy to open from then on)
