My sister is a retired school teacher and likes questions like this. I am certainly not an expert and English grammar from almost 60 years ago was not my favorite subject. But from a purely logical standpoint I believe either could be considered correct. In this case I believe that it "occurred " to him, but with his familiarity with his colleague, it continues to occur to him, and to worry him, and so using “occurs” would be the most appropriate form in this situation . Can’t say that I would disagree. The only thing that puzzles me is the idea that he would be awake and alert in first place!
