Actually not that difficult. A group of hackers stole a bunch of people’s “personally identifiable information” that we’re all constantly assured is kept safe and won’t be sold (doesn’t say won’t be given away or traded, though) and then just filed false tax forms with direct deposit, emptied the accounts and disappeared into the wilds of Russia/’stan/China etc.

Might be able to get some money by hacking the Treasury, but not a tax refund. That’s handled the IRS, which is a separate bureaucracy altogether.
