Large professional garages have floor lifts with restraints that allow you to tip the car 90 degrees to do heavy work on the bottom side. Stuff doesn’t keep falling on the floor or landing in your face.
When I had my neck surgery, I was wheeled into the operating room on my back, they put me out. When I woke I was told that I was put on my stomach and a 45 gallon drum, my hands and legs hung down the side. I am sure it was more surgical than an oil drum but the shape was the same. I guess that is why I was not allowed to see it before the operation.

There is a type of back surgery that can be done by going in through the torso/stomach. A friend of mine had it done a couple of years ago.
