I would take the under bet.

An over-under bet is a two part bet:

Part 1: Betting on when it will be all over for the subject of the bet.

Part 2: Betting on when the subject will be under the ground.

They’re betting they’ll never get over your underwear.

A series of elaborate trusses

Over / Under refers to toilet paper.

Over the hill/Under the sod?

If they’re betting on his lifespan, hopefully the one betting “over” is his doctor!
