If the count is 3-0 and the pitcher has any control left, the next pitch will be right down the middle, swing. Usually what happens is the manager gives the hitter a red light, the next pitch is right down the middle, strike one 3-1. The hitter is poed and swings at anything the next pitch, voila, full count.
