    I wonder if anyone has ever done a study on the costs [e.g., time, money, effort] spent establishing blame (e.g., finding who to blame and proving it) compared to the costs involved in identifying and fixing a problem. Politics aside, as a species we seem much more concerned with personal ego than with cleaning up messes or solving problems.
